Patients who have coverage for orthotics and would like to set up an appointment for a fitting. 

(EHC) Extended Health Coverage Patients:​

Patients who have extended health insurance coverage.

(MVA) Motor Vehicle Accident Patients:

Patients who have been in a recent car accident.

General Patients:

Patients who are experiencing pain and do not have extended health coverage.

Please note, that all patients new and returning must submit an intake form prior to any treatment or service. We will contact you after we have successfully received your intake form.
