Patients who have extended health insurance coverage.

INTAKE FORMS

YOGA intake:

Patients who would like to sign up for yoga classes must submit an intake form prior to attending a class. Patients under 18 must list their name and have a parent or guardian sign the form. 

(MVA) Motor Vehicle Accident Patients:

Patients who have been in a recent car accident.

Custom fit Orthotics:

Patients who have coverage for orthotics and would like to set up an appointment for a fitting.
